• PTA Membership Paper Sign-Up or Sign-Up Online
• Run-A-Thon/Donations or Purchase Online (Donations and pledge forms must be received by 9/10/24 to get your name on our annual Run-A-Thon t-shirt and/or 10/4/24 to be eligible for the prize program.)
• Birthday Book & Banner Club  (deadline is 9/10/24)

• Photo Release Form
• Volunteer form
• PTA Parent/Student Waiver

Please print and complete each of these forms and give them to your child's teacher.
